Default Father's permission to travel

What are the rules when taking a child out of the country with regards to needing the other parent's permission.
Is there a certain form to fill in that he/she needs to sign?

I have flown back and forth with my kids without other parents written permission needed.Is it different if the child is Mexican.?

in the u.s. it depends on if it is specified in the divorce decree....sometimes it will state that parents cannot leave a certain mile radious/state/country without the other parents permission....this would usually take place in a situation where there had been a previous instance where one parent took the child and kept them from the other half for an extended period of time

Any notario will have the form and fill it out for a fee. We've only been asked once but we always have it just in case.

A couple of years ago I took my girlfriend (at the time) and her two minor children to the USA. Their father provided a notarized letter of permission THAT ALSO INCLUDED permission for the mother only to make any and all decisions for any medical reason whatsoever, if needed. I cannot remember for certain but I am pretty sure it was in both Spanish and English. That medical thing seemed very important in the case of an emergency.
